•• Child's birthdate
•• Allergies, medications taken
•• Emergency contact person and phone number
•• Doctor's name and phone number
•• Dentist's name and phone number
•• Credit card information

Also:
• Please try to register multiple children at the same time.

• If you have previously registered one of your children and have returned to register another, please do not register the first child again.

• After completing the payment portion of this form please wait for confirmation your order has been received.
